CAS Activity Proposal Form

Supervisor’s
Name
Mr. Barth
National Honor Society
Every Thursday, 2011 october ~ 2012 may

What is my past experience with this activity?
Before, I was involved in some of the NHS activities such as orphanage visit. Also, I bought some of the foods
that the members were selling to gather money for their activities.
What new learning or skill do I hope to pick up or develop?
I think I will learn how to prepare big projects independently. Even if i would get some help from my supervisor
and friends, i would be the one who would plan my own project, gather it, and successfully finish it. So I am
looking forward to be independent on bigger projects.
How will this experience be different from that of the past?
Last year, when i was not the member of NHS, I just was a helper who was just there to help NHS members to
successfully finish their projects. However, this year, my role has changed. Now, I am the person who organized
the project and held it!

How am I going to know if I am making progress or experiencing growth in this activity?
I always think talking to my NHS members and the supervisor is the best way to find out if i am making progress
or experience growth in thie activity.
Goals What is/are your goal/s for this activity? What do you hope to achieve?
My goals are to really try my best to be a beneficial person in the group that I can be a helpful person. Also, I
want to have many successful projects which can give a better options to our member's projects.
Impact Who will benefit from this activity? How?
I think our whole community and school body can benefit because they can be part of the projects as long as if
they sign up.
Timeline Where, how often, and for how long will the activity take place? (specific dates whenever possible)
Subic, our meetings will be at the school, every Thursday, 2011 oct~ 2012 may

Basketball Journal Entry #4

January

After coming back from the Vacation, all of us had to RUN and RUN to get fit.
We were always complaining but our coach still made us work hard and i think it really paid off.
In ISAC, we could run a lot without getting tired. So, most of us could always run a lot even if we were on the court for a long time. I was to proud of my team. We all worked hard together and we all finished together! I think everything that we have done made us to win a Thrid place.
Personally, i was disappointed by myself because I made a lot of silly mistakes and I couldn't score as much as before. It would have been much better if i could perform better on the court but i couldn't that i was so stressed out. Well, but i think i did way better in the last game so i am proud of myself! I think my last year of Basketball in brent was the best one ever and i will definitly miss it!

Basketball Journal Entry #3

December

We have our Christmas Break this month!
All of us won't be able to practice for about 3 weeks and i think that really can hurt us!
So, Mr.Thompson gave us some work-out scheduels so that we can still work on our fitness!
I hope everyone will work on it so that we won't be DEAD when we come back from the break.
Since we have worked so hard so far, it would be very disappointing if we lose all of them!
So i hope everyone will be commited to this workout andn really work hard, because we MUST get that
championship! I really want it and we really want it!

Basketball Journal Entry #2

November

We've been practicing so hard so far. Our team seems to improve!
Mina is in charge of making sure that we are stretched properly and she also lead us when we are doing Core work-out. I hope this really help us to improve a lot.
Since our team is very small(We only have 12 members), we really had to stay in fit.
We actually started to have some games against teachers. Also, we are part of the manila league that we are going to manila almost every weekend soon! I hope we will win this competition! This must be the great preparation for us to win the ISAC.
I think our team is pretty good in shape because we do not get that much tired when playing games.
It was very tiring when i first started doing core-work out and sprinting. Even if i joined Cross Country, it felt like i am still tired!! :( but i think it still helped me! I was still able to walk..:P
I hope all of us will feel the same way soon that we would never get tired!

Basketball Journal Entry #1

October

Finally the day I have been waiting for came!! :)
Our basketball team was chosen and now we are training for the championship in ISAC
Since this is my last year, i think i really will do my best!
Before, I wasn't motivated this much but now I really feel that this is my last change to play basketball!
As a senior and a IB candidate, i will be the RECORD KEEPER in our team. I will be the one who make the chart for Free-throws and let my teammates to record on the chart and make a visual chart from it. This is to let my teammates to know that they really are improving on their shots!
I am so excited to start the season.

Girls basketball varsity

october ~ january

What is my past experience with this activity?

I have been in the varsity team for almost 3 years, and this is my 4th year playing basketball. I went to ISAC
twice. For about 2 years, I was playing as the post, and I think I will play as a post(blocker) and also the wing this
year.

What new learning or skill do I hope to pick up or develop?

This year, I will try to learn some more skills for the post such as faking and shooting. Also, I want to really learn
how to help my teammates without screaming badly! I want to be more nice to my teammates but still be a good
leader. Another thing that I hope to develop is to really communicate more. In my oppinion, communication is
the most important thing that will make a good team-work. So, I will try my best to really communicate with other
post players and also other teammates to creat a better team-work and bring the team together.

How will this experience be different from that of the past?

Unlike last year, I am keeping track of our team's records. My plan is to record the data of the free throws at
practice right before the practice and make a graph from it so that our team can actually see the improvement
or what we have to improve. Also, I will try to get some data from our games so that we would know what to
improve on and what to work on. Another thing that I would do this year is to come up with the names for our
new plays. For example, Mina and I decided to call our offense break out as "Key." These are the new things
that I am planning to do this year, and I hope this would help me but also our teammates.

How am I going to know if I am making progress or experiencing growth in this activity?

I will can know if I grow with this activity by talking to the coaches. I think they are the ones who know the best
about myself progress on the court. Also, if i can see my teammates improving by all the table charts that i will
going to make, i will find out if what i am doing really is helping them to improve and also helping me to improve.

Goals What is/are your goal/s for this activity? What do you hope to achieve?

My goal is to be a leader of the team. I want to communicate more and help my teammates to improve more and
more. Also, my another goal is to really do my work, which is to keep track of our defense, and recording the
scores. I want to keep my records organized and accurate so that my teammates can rely on the results.

Impact Who will benefit from this activity? How?

I will benefit because I will learn to organize and analyze my results. Also, this season is the time when I get
to talk to my teammates a lot so I will learn how to help and encourage my teammates without hurting their
feelings. Also, I believe that my teammates will benefit too because they will get the records about how good
they are or what to improve on.

Timeline Where, how often, and for how long will the activity take place? (specific dates whenever possible)

Gym, Monday to Friday, about 2 hours per day, from october to january.
